fix: improve init-bootstrap vault selection and user identification
- Add explicit handling for multiple vaults found - Require confirmation before importing any vault - Better prompts explaining why we ask for name and research permission - Add disambiguation prompts to find the right person - Clear instructions to never proceed without explicit confirmation
